Office of the Registrar

Prospective students must include the following forms in a complete application package. Please do not send forms independant of one another as this will delay processing of your child's paperwork. Each form is available on King's Prep website, however, you may pick up a hard copy of selected forms from the Office of the Registrar Mondays & Wednesdays from 9:00 a.m. - 2:00 p.m. without appointment.

Please ensure that a recent photo of your child accompanies their completed application.

Following is a list of documents that must be included in the KPA application for admission:

• KPA Student Application (4 pages long)
• Immunization Form 2.3
• Parent Contract
• Open House Registration/Open House Confirmation
• Recent Photo
• Extra-Curricular Activity Interest Form
• Letter of Recommendation
• Hearing & Vision Screening Form
• Most Recent Transcripts (Grades 2 or higher)
• Statement of Faith (Optional)
• Application Fee

Completed applications can be mailed to the Office of the Registrar

Applications must be postdated by the deadline, not received by the deadline.
